Unfortunately, the 1970 \u201cActs\u201d aren\u2019t available online. It has been modified 15 times. The report shall list separately all loans, and for each loan, shall give:\n\n 1. The date the loan was made;\n\n 2. The name and address of the person making the loan and any person who is a\n co-borrower, guarantor, or endorser of the loan;\n\n 3. The amount of the loan;\n\n 4. The date and amount of any repayment of the loan; and\n\n 5. For any loan or part of a loan that is forgiven by the lender, the amount\n forgiven listed as both a contribution and loan repayment.\n\nG. The State Board shall provide for a “no activity” report that may\nbe filed for any reporting period in which the filer has no activity to report.\n\nH.
