6.<\/span> A description of the specific services to be offered by the applicant including such elements as may be specified by the Department<\/span> in regulations; <\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t

7.<\/span> A staffing plan, including information regarding employee credentials and job descriptions, containing such elements as may be specified by the Department<\/span> in regulations; <\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t

8.<\/span> Operating policies that contain such elements as may be specified by the Department<\/span> in regulations; and <\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t
