A managed care health <span class=\"dictionary\">insurance<\/span> plan, as defined in Chapter 58 (&#xA7; <a class=\"law\" title=\"Definitions\" href=\"\/38.2-5800\/\">38.2-5800<\/a> et seq.) of this title, may require such health care professional to be a member of the plan&#8217;s provider network; provided that such network includes sufficient health care professionals who are qualified by specific education, experience, and credentials to provide the covered benefits described in this section. <a id=\"paragraph-309084\" class=\"section-permalink\" href=\"https:\/\/vacode.org\/38.2-3418.10\/#C\"><i class=\"fa fa-link\"><\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t<section id=\"D\"><p><span class=\"prefix-number\">D.<\/span> No <span class=\"dictionary\">insurer<\/span>, corporation, or health maintenance organization shall impose upon any <span class=\"dictionary\">person<\/span> receiving benefits pursuant to this section any copayment, fee or condition that is not equally imposed upon all individuals in the same benefit category, nor shall any <span class=\"dictionary\">insurer<\/span>, corporation or health maintenance organization impose any policy-year or calendar-year dollar or durational benefit limitations or maximums for benefits or services provided under this section. <a id=\"paragraph-309085\" class=\"section-permalink\" href=\"https:\/\/vacode.org\/38.2-3418.10\/#D\"><i class=\"fa fa-link\"><\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t<section id=\"E\"><p><span class=\"prefix-number\">E.<\/span> The requirements of this section shall apply to all <span class=\"dictionary\">insurance policies<\/span>, <span class=\"dictionary\">contracts<\/span> and plans delivered, issued for delivery, reissued, or extended on and after July 1, 2000, or at any time thereafter when any term of the policy, <span class=\"dictionary\">contract<\/span> or plan is changed or any premium adjustment is made. <a id=\"paragraph-309086\" class=\"section-permalink\" href=\"https:\/\/vacode.org\/38.2-3418.10\/#E\"><i class=\"fa fa-link\"><\/i><\/a><\/p><\/section>\n\t\t\t\t\t\t<section id=\"F\"><p><span class=\"prefix-number\">F.<\/span> This section shall not apply to short-term travel, accident only, or limited or specified disease policies or <span class=\"dictionary\">contracts<\/span>, nor to policies or <span class=\"dictionary\">contracts<\/span> designed for issuance to <span class=\"dictionary\">persons<\/span> eligible for coverage under Title XVIII of the Social Security Act, known as <span class=\"dictionary\">Medicare<\/span>, or any other similar coverage under <span class=\"dictionary\">state<\/span> or federal governmental plans. <a id=\"paragraph-309087\" class=\"section-permalink\" href=\"https:\/\/vacode.org\/38.2-3418.10\/#F\"><i class=\"fa fa-link\"><\/i><\/a><\/p><\/section>","plain_text":"                                 CODE OF VIRGINIA\n\nCOVERAGE FOR DIABETES (\u00a7 38.2-3418.10)\n\nA. Each insurer proposing to issue an individual or group hospital policy or\nmajor medical policy in this Commonwealth, each corporation proposing to issue\nan individual or group hospital, medical or major medical subscription contract,\nand each health maintenance organization providing a health care plan for health\ncare services shall provide coverage for diabetes as provided in this section.\n\nB. Such coverage shall include benefits for equipment, supplies and in-person\noutpatient self-management training and education, including medical nutrition\ntherapy, for the treatment of insulin-dependent diabetes, insulin-using\ndiabetes, gestational diabetes and noninsulin-using diabetes if prescribed by a\nhealth care professional legally authorized to prescribe such items under law.\nAs used herein, the terms &#8220;equipment&#8221; and &#8220;supplies&#8221;\nshall not be considered durable medical equipment.\n\nC.
