						<section><p>The <span class="dictionary">State Board</span> of Medical Assistance Services may require in its Plan for Medical Assistance that any application, statement, or form filled out by suppliers of medical care under medical assistance shall contain or be verified by a written statement that it is made under the penalties of <span class="dictionary">perjury</span> and such declaration shall be in lieu of any <span class="dictionary">oath</span> otherwise required, and each such paper shall in such event so state. Any <span class="dictionary">person</span> who knowingly and willfully makes or subscribes any such papers or forms containing any false or misleading information shall be guilty of a Class 4 <span class="dictionary">misdemeanor</span>.</p></section></text><history>1981, c. 255; 1984, c. 781.</history><metadata></metadata></law>
