						<section id="A"><p><span class="prefix-number">A.</span> An application for a certificate shall (i) be in writing, in such form as the <span class="dictionary">Commission</span> prescribes, (ii) be verified under <span class="dictionary">oath</span>, (iii) be supported by such information, data, and records as the <span class="dictionary">Commission</span> may require, and (iv) include an <span class="dictionary">operating plan</span>. <a id="paragraph-250820" class="section-permalink" href="https://vacode.org/6.2-1015/#A"><i class="fa fa-link"/></a></p></section>
